Hospital Discharge White Paper
Twenty five percent of hosptial readmissions could be avoided with timely coordination of care around a shared discharge care plan. Primary care providers (PCPs) who participate in a medical home can be incented to work with a discharge care plan that notifies patients and providers to items in the plan that need follow-up, triggers workflow processes around these tasks to ensure they are completed in a timely manner and coordinates the care team to collaborate around the care plan. Community Health Dialtone (a technology platform) can ensure that this happens in a cost effective, scalable way.
